Medical Release: As part of the registration process, a legal guardian must agree and consent to the following: I, as a legal guardian of the student being registered, herby release the camp, its employees and Midland Christian School from all claims of injury or illness that may be sustained by, perceived or otherwise, participation in Midland Christian Summer Athletic Camps. I authorize camp personnel to seek treatment for my child if they deem a situation as a medical emergency. The finalization and payment of this online registration serves as your digital signature and agreement to (1) the above stated Medical Release, (2) permission for your child to participate and (3) acknowledgment that he/she is physically able to participate in MCS registered camp activities.
